Key Responsibilities:

  • Develop and implement individualized treatment plans to address a variety of diseases and impairments
  • Work collaboratively with a multidisciplinary team to ensure a comprehensive and supportive rehab experience for patients
  • Utilize progressive treatment modalities to address complex patient needs
  • Conduct in-house therapy sessions and provide local management support

Requirements:

  • Graduate of an accredited Therapy program
  • Licensed in the state of the SNF you will be working
  • 3 C's (Certifications)
  • Previous rehab SNF experience preferred (1 year experience preferred)
