Job Overview

This role offers an exciting opportunity to make a difference in the lives of patients and their families as a Licensed Practical Nurse (LPN) in Home Health. We strive to provide a work environment that supports your growth and well-being.

Responsibilities

As a skilled LPN, you will be responsible for providing high-quality patient care and performing technical skilled care in the comfort of the patient's home. Your duties will include:

  • Implementing the plan of care in accordance with state laws governing nursing practice and in coordination with the healthcare team.
  • Instructing patients and their families on proper care and maintenance of a healthy environment.
  • Observing, recording, and reporting patient responses to treatment and changes in condition daily.
Requirements

To succeed in this role, you must have:

  • A current LPN licensure in the state where you will be practicing.
  • A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification.
