Clinical Case Manager

The Clinical Case Manager plays a vital role in providing alcohol, drug, and mental health treatment to individuals in the Adult Outpatient Clinics. This position involves evaluating the mental health needs of patients, providing group counseling and individual therapy as needed, and coordinating case management services to further treatment goals. The ideal candidate will possess a master's degree in human services, psychology, counseling, social work, or a directly related field, and have two years of professional experience working with dual diagnosed patients.
Key Responsibilities:
- Identify dually diagnosed patients and coordinate effective treatment interventions.
- Determine when a patient needs to be referred for psychiatric evaluation and coordinate appointment schedules for the psychiatric team.
- Provide counseling, such as group or individual interventions, as necessary.
- Contribute to the development of person-centered care plans.
- Maintain current and accurate records as required by policy.
- Collaborate with external stakeholders, such as the Department of Human Services and Criminal Justice systems.
- May conduct abstinence monitoring.
Work Environment:
This role is expected to work onsite and is located in Denver and Aurora, CO. Qualifications:
Minimum Qualifications:
- A master's degree in human services, psychology, counseling, social work, or a directly related field from an accredited institution.
- Two years of professional experience working with dual diagnosed patients.
- Possess a valid registration in good standing with the Colorado Department of Regulatory Agencies (DORA) as a Licensed Professional Counselor (LPC), Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).
Preferred Qualifications:
- Three years of professional experience working with dual diagnosed patients.
- Experience with and knowledge of running evidence-based counseling groups with dually diagnosed patients.

- Well-developed knowledge and skills in care coordination for the dually diagnosed population.
- Ability to apply critical thinking and conflict resolution skills using creative approaches.
- Strong organizational and time management skills to meet deadlines.
- Ability to work with diverse patient populations and be nonjudgmental and respectful of others' beliefs, values, and culture.
- Ability to comply with established rules, policies, and procedures.
- Ability to effectively and efficiently manage a diverse workload in a fast-paced, rapidly changing regulatory environment.
- Proficient in Microsoft Office programs and ability to learn software programs.
- Ability to communicate effectively, both in writing and orally.
- Ability to establish and maintain effective working relationships with employees at all levels throughout the institution.
- Outstanding customer service skills.
- Demonstrated commitment and leadership ability to advance diversity and inclusion.
- Ability to establish boundaries with clients and exercise discretion (e.g., patient confidentiality).
