Job Description: We are seeking a skilled RN Transitional Care Coordinator to join our team in Portage, Ohio. This is a part-time position with flexible hours, ideal for those who want to maintain a work-life balance. The successful candidate will be responsible for coordinating the transition of patients from acute care settings to post-acute care facilities or home environments.
Responsibilities:
- Develop and implement individualized care plans for patients transitioning from acute care settings
- Collaborate with healthcare teams to ensure seamless transitions and optimal patient outcomes
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
- Maintain accurate and up-to-date records of patient information
- Participate in ongoing education and training to enhance knowledge and skills
Qualifications:
We require a graduate degree in nursing from an accredited institution and a current Registered Nurse (RN) license in the State of Ohio. A Bachelor's degree is preferred. At least one year of clinical nursing experience is required, with experience in case management/discharge planning in an acute care setting highly desirable. Experience and knowledge of utilization management, case management, post-acute transitions, and home health are also advantageous.
Salary Range: The estimated salary range for this role is $65,000-$80,000 per annum, depending on experience. We offer comprehensive benefits, including medical, dental, and vision coverage, as well as a 401(k) plan.
