Salary
$105.00-$110.00 per completed assessment, depending on the county.
Job Description
We're seeking a skilled Nurse Practitioner to provide comprehensive health assessments, medical history reviews, diagnoses, treatments, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols. This role involves working collaboratively with physicians, case managers, social workers, family members, caregivers, and other healthcare professionals as needed.
Requirements
To be successful in this position, you'll need:
- A Master's degree in Nursing or equivalent experience and satisfactory completion of NP licensure
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain it
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
- A valid BLS, ACLS, or CPR certification
- At least one year of experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health record software
- Excellent verbal and written communication skills
