Job Summary

We are seeking a highly skilled Care Manager RN to join our team at Providence Service. As a key member of our care management team, you will be responsible for managing patient populations with chronic illness and/or multiple co-morbidities.

Key Responsibilities
  • Manage patient populations with chronic illness and/or multiple co-morbidities
  • Work in coordination with multiple PCPs and serve as the central link to an identified population of patients
  • Develop and implement care plans to improve patient outcomes
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Provide education and support to patients and families to promote self-management and empowerment
Requirements
  • Graduation from an approved, accredited school of Nursing
  • Washington Registered Nurse License
  • 2 years Work experience in an RN role
  • Experience managing complex patients in an ambulatory setting
