With a focus on patient-centered care, you will work closely with a team of physicians and other healthcare professionals to develop and implement personalized care plans.

Responsibilities of a Home Health RN Case Manager include:
- Providing home-based care to patients with illnesses, injuries and chronic diseases
- Working with a team of physicians and overseeing LPNs, CNAs and aides providing patient care
- Examining patients, doing assessments, documenting progress and reporting changes to their physician
- Educating patients on their plan of care, prescribed medication, therapy, diet and exercise
- Coaching family members on the patient's plan of care, medication and home safety
- Ensuring goals are met and coordinating discharge from services

To be successful as a Home Health RN Case Manager with Interim HealthCare, you will need:
- Registered Nurse (RN) with active, unrestricted license in the State of Michigan – BSN not required
- At least one (1) year of clinical experience as a Registered Nurse
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
