About the Role: We are seeking a skilled and experienced Physical Therapist to join our team as a Home Health Float Traveler. This is a salaried position that requires the ability to travel to various locations, supporting the delivery of high-quality physical therapy services to patients in their homes.
Responsibilities:
- Administer physical therapy to patients on an intermittent basis in their place of residence.
- Develop and evaluate the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Participate in patient, caregiver, and family counseling and caregiver education.
- Communicate with all physicians involved in the plan of care and other healthcare practitioners (as appropriate) related to the current plan of care.
- Supervise Physical Therapy Assistants and Home Health Aides as needed.
Requirements:
- Graduate of an accredited School of Physical Therapy.
- Current license as a Physical Therapist in the state of practice.
- At least one (1) year of clinical experience, preferably in a clinical supervisory role.
- Good communication and organizational skills.
- Ability to establish and maintain working relationships with all segments of staff as well as professionals and lay persons.
- Current CPR certification.
- Active driver's license in the state of residence with evidence of valid automobile insurance and acceptable MVR.
Work Environment: This position requires the ability to travel to various locations, including patient homes, and may involve exposure to allergens and other various conditions. The physical therapist must be able to adapt to different environments and possess the ability to work independently with minimal supervision.
