Job Summary:The successful candidate will coordinate the delivery of health care services for adults with disabilities residing in residential facilities. This role will cover various group home locations in the Hudson Valley, primarily the Poughkeepsie and Fishkill area.
Responsibilities:- Coordinate the delivery of routine and specialty health care services for adults with disabilities.
- Assess and establish health care outcomes for individuals receiving services, updating assessments as necessary.
- Provide guidance and training to direct care staff to ensure proper delivery of health care services.
- Develop and implement health care plans, educating individuals, caregivers, and families on the plan.
- Evaluate the effectiveness of preventive and ongoing health care, making recommendations and referrals as needed.
- Act as a liaison to communicate health information between team members, families, and other service providers.
- Provide on-call triage coverage and be prepared to visit sites as needed.
- Complete relevant medical documents, assist in developing policies and procedures, and establish working systems.
- NYS RN License, BSN preferred.
- Prior nursing experience and/or experience working with individuals with developmental disabilities preferred.
- Excellent oral and written communication skills.
- Able to work flexible hours and/or days.
- Four weeks' vacation, personal, and holiday days.
- Tuition reimbursement.
- License renewal and conference/workshop reimbursement.

- 403(b).
- Potential to qualify for loan forgiveness under the Public Service Loan Forgiveness Program.
