Job Title: Physical Therapist - Home Care Specialist

Job Summary:

Metro Physical & Aquatic Therapy is a comprehensive multidisciplinary organization that offers a range of healthcare services, including physical, occupational, and speech therapy. We are seeking a motivated and passionate Physical Therapist to join our team as a Home Care Specialist.

Responsibilities:

  • Develop and implement individually designed programs to restore overall function and independence in the home setting.
  • Plan, prepare, and carry out skilled therapy to patients to meet their specific needs.
  • Collaborate with physicians, family members, and other healthcare professionals to ensure effective care and treatment.
  • Provide timely and accurate documentation of visits and communicate discharge plans with family members and other team members.
  • Instruct family members, PTA's, Home Health Aides, and other caregivers in appropriate physical therapy techniques.

Requirements:

  • Graduate of a Physical Therapist Program.
  • NY Physical Therapist License or eligible.
  • Experience in home health care is preferred but not required.
  • Certifications Required: CPR/First Aid.
