About the Role

We are seeking a skilled Care Coordinator to join our Behavioral Health Division. As a key member of our team, you will provide essential support and guidance to adults with psychiatric disabilities, helping them navigate their medical conditions and access community resources.

Responsibilities:
  • Medical Education and Consultation: Provide education and consultation to consumers and staff regarding medical health, medication, and medical topics.
  • Training and Development: Develop and conduct groups and trainings for consumers and staff in general areas of health and wellness, including nutrition, exercise, and smoking cessation.
  • Community Resource Assistance: Assist consumers with accessing community resources that will support and assist with addressing needs and health wellness efforts.
  • Medical Oversight: Provide oversight, guidance, and assistance to consumers with understanding their medical conditions, meet with consumers and staff to discuss medical diagnoses and conditions, and perform direct medical care as appropriate and ordered by physician.
Schedule and Location

This full-time position is available Monday through Friday, 9am-5pm, at our Danvers, MA location.
