Job Summary

We are seeking a skilled and compassionate Private Duty Nurse - LVN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes. You will work closely with our patients, their families, and our healthcare team to develop and implement personalized care plans that meet their unique needs.

Responsibilities
  • Implementation of Physicians' Prescribed Plan of Care
  • Conduct patient assessments and coordinate care with our healthcare team
  • Administer prescribed medication, treatments, and therapies as ordered by physicians
  • Document care provided in our electronic system of record
  • Provide health education and training to patients and their families
  • Accept and write verbal orders from physicians as required, and obtain co-signature of Case Manager on all verbal orders
  • Follow up with physicians, patients, families, and our healthcare team on patient problems, concerns, and changes in patient condition
Qualifications
  • Active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
  • Reliable transportation
