    • Collaborate with the healthcare team to optimize medication regimens
  • Patient Education and Support
    • Provide patient education on disease management, medication adherence, and self-care
    • Support patients and families in navigating the healthcare system
    • Collaborate with community resources to ensure seamless transitions of care
  • Collaboration and Communication
    • Collaborate with the healthcare team to achieve optimal patient outcomes
    • Communicate effectively with patients, families, and healthcare providers
    • Participate in interdisciplinary rounds and care conferences
Requirements
  • Graduate from an accredited School of Nursing
  • Current License to practice as a Registered Professional Nurse in New York State
  • Minimum 1-year Cardiac Telemetry experience required, and Critical Care (ICU/CCU) experience preferred
  • BLS/ACLS Required
Preferred Qualifications
  • Bachelor's Degree in Nursing
  • Specialized certifications as needed
