Novant Health is seeking a highly skilled Clinical Pharmacist to join our acute care pharmacy team. As a key member of our team, you will play a critical role in delivering optimal medication management across all dimensions of care.
Responsibilities- Provide comprehensive clinical pharmacy services for patients on the medical oncology unit or service and palliative care unit.
- Order verification, rounds, oncology treatment plan support, discharge counseling, and pharmacokinetic and therapeutic consultation.
- Collaborate with healthcare providers within decentralized patient care areas to provide patient-centered care.
- Makes appropriate evidence-based, patient-centered medication recommendations.
- Participates in the management of medical emergencies.
- Provides discharge medication review, reconciliation, and counseling as appropriate.
- 4 Year/Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred.
- Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
- Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required.
- Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required.
