We are looking for a highly skilled and compassionate Compassionate Healthcare Associate to join our Home-based Medical Care team in Mc Dowell, NC.

Responsibilities and Qualifications

">
  • Establish effective relationships with patients, caregivers, and healthcare providers.
  • Perform initial comprehensive assessments and provide ongoing care, including visits to address changes in condition.
  • Develop and update patient management care plans as needed.
  • Consult with Physician Leaders and coordinate with Primary Care Physicians (PCPs) when available.
  • Perform basic procedures of episodic nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, and basic wound care.
  • Participate in HEDIS measure campaigns and quality initiatives to ensure the highest standards of care.
  • Participate in clinical after-hours on-call program on a rotational basis.
