Job Summary:

The RN Case Manager plays a vital role in facilitating and coordinating patient care to ensure quality and continuity. This position requires collaboration with the interdisciplinary team to implement care plans and transition strategies, ensuring desirable patient outcomes and efficient resource utilization.

Key Responsibilities:
  • Develops individualized care plans, prioritizing goals that consider patient and caregiver preferences and involvement.
  • Makes outbound calls to assess patient health gaps and barriers in treatment plans.
  • Coordinates care for members, making referrals to outside sources as needed.
  • Ensures patients discharged from hospital or emergency department receive necessary services and resources.
  • Provides approved general health information and guidance to public callers.
  • Documents all workflows in EPIC system for tracking and quality assessment.
  • Provides patient education to assist with self-management of wellness and disease.
  • Educates members on chronic and acute disease processes.
  • Forms partnerships with providers to achieve quality and cost management objectives.
  • Tracks and monitors enrolled members throughout the continuum of care.
  • Interacts with LCMC Healthcare Partners leadership and/or physician group partners to discuss clinical questions and concerns.
  • Participates in case conferences with the interdisciplinary team to strategize and develop member-specific care plans.

Requirements:
  • 2-3 years of Nursing experience required.
  • Bachelor's Degree in Nursing or equivalent.
  • Certification in Basic Life Support Health Care Provider.
  • Registered Nursing License from the Louisiana State Board of Nursing.
  • Preferred: Certification as a Certified Case Manager from the Commission for Case Manager Certification.
