We are seeking a highly skilled Clinical Pharmacist Specialist to join our team at the Ralph H. Johnson VA Medical Center. As a key member of our Pharmacy Service, you will provide evidence-based pharmaceutical care services to patients in various clinical areas, including the Medicine Intensive Care Unit, Surgical Intensive Care Unit, and Emergency Department.
Key Responsibilities- Provide clinical coverage in several wards/clinics as assigned.
- Offer expert knowledge on pharmacology, therapeutics, drug-drug interactions, medication safety, compatibility guidelines, and aseptic techniques.
- Perform comprehensive assessments of patients upon admission to the hospital and as necessary during hospitalization for appropriateness of pharmacologic therapy.
- Design, implement, monitor, and document pharmaceutical care plans.
- Participate in the education and training of new employees or employees introduced to selected practice areas.
- Assume an ownership role for administrative activities related to advanced practice setting.
- Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
- Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
- At least 1 year of experience equivalent to the next lower grade level (GS-12).
- Completion of a PGY-1 Pharmacy Residency or 3 years work experience in an inpatient pharmacy setting.
- Competitive salary and regular salary increases.
