At Genesis Healthcare LLC, we are seeking a skilled Care Transitions Nurse to join our team. As a key member of our care team, you will be responsible for developing strong relationships with hospital providers, hospitalists, physicians, and home health professionals to grow patient referral volume through the delivery of market-leading patient outcomes.

Key Responsibilities:

  • Expand customer base by increasing nursing center occupancy and skilled mix from assigned hospital accounts and/or mining unassigned hospital accounts.
  • Make patient acceptance and placement decisions on behalf of Genesis and communicate decisions to referral sources based on published performance standards.
  • Ensure safe and effective transfers of patients across the care continuum serving as a bridge between the hospital and nursing centers, staff, and the patient and/or family with the objective to reduce rehospitalizations and improve customer satisfaction.

Requirements:

  • RN License in state in which employment occurs and willingness to obtain licensure in additional states as requested.
  • Valid driver's license.
  • Seven years plus of recent clinical experience.
  • Available 24 x 7 to meet hospital demand.
  • Experience coaching clinical teams.
  • Required case management experience.
  • Experience in business/development preferred.
  • Excellent written, verbal, and listening communication abilities.
  • Prior experience in utilization review or discharge planning preferred.
  • Certifications: CCM, CRRN, or CIRS preferred.
