Mobility Rehabilitation Specialist

We are seeking a skilled Physical Therapist to join our team in the Greeley area. This is a full-time or part-time opportunity with a minimum expectation of 5 visits or 10 hours per week.

The successful candidate will have a graduate degree from an accredited Physical Therapist program and an active PT license in Colorado. A minimum of 1 year of physical therapy experience, ideally in home healthcare, is required.

The ideal candidate will possess good clinical judgment, strong interpersonal skills, dexterity, and compassion. They will also be knowledgeable of state and federal home health regulations and able to pass a background check and drug test.

Job Responsibilities:

• Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
• Work as part of a home health team focused on the patient's plan of care and goals
• Observe movements, conduct assessments, document progress, and modify care plans as needed
• Assist patients with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises, and self-care

Estimated Annual Salary: $97,000 - $120,000 based on average annual earnings for a full-time Physical Therapist in the Timnath, CO area.
