We are seeking a skilled Pulmonary Disease Navigator to join our team at Novant Health. The successful candidate will work directly with patients diagnosed with chronic pulmonary diseases, ensuring proper care and education throughout the continuum of care.
Key Responsibilities- Act as a liaison between patients, families, and providers to ensure proper length of stay and safe discharge.
- Ensure quality metrics specific to disease process are met.
- Promote compliance with management of chronic pulmonary diseases.
- Provide referrals for follow-up testing and/or to other disciplines and specialties when appropriate.
- Graduate from an accredited school for Respiratory care, required.
- Minimum of three years in acute care respiratory therapist experience, required.
- Currently registered by National Board Respiratory Care, required.
- Additional skills: ability to successfully complete generic and department-specific competency assessment, current practice in area of responsibility, and application of safety and infection control policies and procedures.
