Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. As a key member of our clinical team, you will conduct comprehensive health assessments, provide medical guidance, and collaborate with healthcare professionals to ensure high-quality patient care.

• Conduct in-depth health assessments, medical history reviews, and diagnosis to identify potential health risks and develop personalized care plans.
• Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless patient care and coordination.
• Provide education and guidance to patients and their families on healthy lifestyle choices, disease management, and medication adherence.
• Work in a variety of settings, including home visits, skilled facilities, and telehealth visits, to meet the diverse needs of our patients.

• Master's degree in Nursing or related field, or equivalent experience and licensure.
• Current RN and NP licensure, with prescription authority or ability to obtain.
• Board certification in a Matrix-approved specialty.
• Minimum of 1 year experience as a Nurse Practitioner, with strong clinical skills and patient communication abilities.
• Excellent computer skills and familiarity with electronic health records.
• Ability to travel to patient locations, with a valid driver's license and adequate automobile insurance.
