About the Role

We are seeking a highly skilled Physical Therapist to join our team at Carle Health. As a Physical Therapist, you will play a vital role in evaluating and treating patients of all ages, modifying treatment plans as necessary to ensure optimal outcomes.

Responsibilities
  • Design and implement individualized treatment plans that integrate patient goals, treatment objectives, and discharge planning.
  • Educate patients, family members, caregivers, and other healthcare professionals on relevant therapy techniques and best practices.
  • Conduct thorough evaluations to determine physical therapy diagnoses and assess the effectiveness of treatment interventions.
  • Collaborate with multidisciplinary teams to ensure seamless care transitions and optimize patient outcomes.
Requirements
  • Bachelor's degree in Physical Therapy or equivalent from an accredited institution.
  • Current licensure as a Physical Therapist (PT) in Illinois upon hire and Basic Life Support (BLS) certification within 30 days.
  • New graduates welcome to apply.
