Key Responsibilities- Coordinate the care team to develop and implement individualized care plans
- Provide primary nursing care to patients in various settings, including home and skilled nursing facilities
- Assess patient needs and develop plans to manage symptoms and improve quality of life
- Teach caregivers skills to support patient care at home
- Collaborate with physicians and other team members to ensure comprehensive care
- Maintain accurate and detailed clinical records
- Supervise hospice aides and certified nurse aides
