POSITION: REGISTERED NURSE CASE MANAGER (RN) - FULL TIME

JOB PURPOSE:
The Registered Nurse Case Manager is responsible for evaluating referrals for suitability and facilitating the admission process to ensure a smooth transition for patients and their families into the hospice program. This role also involves managing patient care to guarantee effective symptom management, education for patients and families, medication oversight, coordination of hospice services, and maintaining excellent communication with the patient's healthcare provider.
KEY RESPONSIBILITIES:
Coordinate the interdisciplinary care team for assigned patients and oversee the execution of the care plan. Deliver primary nursing care to hospice patients, regardless of their location. Take full responsibility for the professional management of care for assigned hospice patients dealing with terminal illnesses. Educate primary caregivers on essential skills needed for patient care at home. Collaborate closely with attending physicians and team members to establish effective pain and symptom management strategies. Conduct nursing assessments and patient admissions. Maintain clinical records in compliance with federal, state, and agency regulations. Supervise hospice aides and certified nursing assistants, ensuring regular supervisory visits. Assume on-call responsibilities as designated. Engage in Performance Improvement Programs. Participate in and lead in-service training, team meetings, and staff conferences. Provide skilled nursing care to patients and families as per the care plan. Contribute to the development of the interdisciplinary care plan for patients and families. For patients in skilled nursing facilities, manage symptoms related to terminal illnesses. Verify that caregivers in skilled nursing facilities are executing the care plan as intended. Ensure integration of the care plan with both hospice and skilled nursing facility teams. For patients receiving private home care, supervise certified nursing aides and companions. Perform additional duties as assigned by the Director of Health Services or Administrator.KNOWLEDGE, SKILLS, ABILITIES:

Flexibility in work scheduling. Commitment to hospice principles and standards. Capacity to interact with diverse age groups. Participate in agency evaluations and necessary reporting. Engage in continuing education to stay updated in the field and maintain licensure. Attend mandatory training sessions. Respect patients' rights to fair treatment, privacy, and individuality. Adhere to corporate compliance protocols. Report any job-related hazards and follow established safety regulations.
