Job Title: Hospice Nurse - Baylor Shift

Job Summary:

Hartford HealthCare at Home is seeking a compassionate and skilled Hospice Nurse to join our team. As a Hospice Nurse, you will provide high-quality, patient-centered care to individuals with life-limiting illnesses in the comfort of their own homes.

Responsibilities:

  • Conduct comprehensive assessments to establish goals for symptom management and end-of-life care
  • Develop and maintain therapeutic relationships with patients and their families
  • Collaborate with the interdisciplinary team to address patients' needs holistically
  • Utilize state-of-the-art technology to document skilled services provided

Requirements:

  • Registered Nurse with a valid CT RN license
  • One year of RN experience; Homecare or Hospice experience preferred
  • Valid driver's license and reliable transportation required
  • Computer proficiency required
