About the Role

We are seeking an experienced Hospice Clinical Care Specialist to join our team as a Hospice Clinical Social Worker in Sacramento County and surrounding areas. This part-time position offers flexible scheduling options, including PRN shifts, with a minimum of 4 shifts per month required. You will work closely with our interdisciplinary team to deliver high-quality care to patients and their families.

Key Responsibilities:
  • Provide compassionate support to patients and their families
  • Collaborate with physicians, nurses, and other healthcare professionals to deliver coordinated care
  • Develop and implement individualized care plans that meet the unique needs of each patient

Requirements

To be considered for this role, you will need:
• A Master's degree in Social Work or a related field
• Valid licensure/registration in the state(s) of agency operation if required
