We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our Hospice team as a Patient Care Coordinator. In this role, you will provide high-quality, patient-centered care to individuals and their families during end-of-life transitions.
Key Responsibilities- Provide skilled nursing and palliative care to assigned hospice patients under the direction of a Registered Nurse.
- Work cooperatively with the Interdisciplinary Hospice Team to develop and implement individualized care plans.
- Perform prescribed medical treatments, including pain management and symptom control.
- Provide nursing observations, education, and support to patients, families, and caregivers.
- Participate in care plan development and implementation.
- Graduate of an accredited school of vocational/practical nursing.
- Current CA license.
- At least one year experience as an LVN/LPN.
- Demonstrated competency in performance of skilled nursing functions.
- Ability to work with dying patients and their families.
- A small service area with a close-knit team.
