LHCSA Registered Nurse

FreedomCare, LLC is seeking a compassionate and empathetic LHCSA Registered Nurse to join our team. As a key member of our Licensed Home Care team, you will play a vital role in assessing patients' current needs and providing high-quality care and services.
Responsibilities:- Provide care and services according to scope and standards of Nursing Practice and adhere to the Code of Ethics for Nurses.
- Respect client and employee rights and privacy, ensure the security of protected information, and practice in an ethical manner.
- Follow agency policies and procedures regarding infection control measures.
- Assess and regularly reassess clients' needs and their response to care, treatment, and education.
- Develop, implement, evaluate, and revise clients' plans of care, including physician plans of treatment, problem lists, nursing care plans, and aide plans of care.
- Complete all client documentation, including written records, reports, and notes, in accordance with agency policy and procedure.
- Actively coordinate clients' care through communication and written documentation with physicians, case managers, supervisors, other disciplines involved in clients' care, labs, equipment, or supplies, and community resources.
- Supervise ancillary staff involved in clients' plans of care.
- Provide education to clients and families through assessment of learning needs, development of teaching objectives and plans, implementation of teaching plans, evaluation, and revision of teaching plans as needed.
- Interpret the agency's philosophy and objectives to clients, families, and others.
- Participate in the agency's Performance Improvement Program.
- At least an Associate's Degree and current NYS License in good standing.
- 3+ years of Nursing experience.
- Ability to travel within the 2 boroughs.
- Current knowledge of state and federal home health regulations and community service systems.
- Proficient in Microsoft (Word, Excel, Outlook).
- Excellent verbal and written communication, organizational, and time management skills.
- Bilingual in English and one of the following languages: Spanish, Russian, Chinese, Korean, and Bengali.
- Experience in Home Care, Geriatrics, and Case Management; field experience assessing patient care.
