Job Summary:

We are looking for a dedicated and compassionate Registered Nurse to join our team as a Case Manager. The successful candidate will be responsible for coordinating patient care, facilitating communication between healthcare providers, and ensuring seamless transitions between care settings.

Key Responsibilities:
  • Coordinate patient care plans, ensuring timely and effective delivery of services
  • Communicate with patients, families, and healthcare providers to ensure informed decision-making
  • Facilitate transitions between care settings, including hospital to home or other facilities
  • Collaborate with interdisciplinary teams to optimize patient outcomes
  • Document patient information and care plans in accordance with department standards
Requirements:
  • Current and valid RN license
  • Minimum 2 years of experience in Med/Surg, Long Term Acute Care, or similar field
  • Strong analytical, data management, and PC skills
  • Excellent interpersonal communication and negotiation skills
  • Knowledge of third-party payer systems and levels of care
Preferred Qualifications:
  • Prior experience in Homecare, Emergency Department, or ICU
  • BSN or Case Manager Certification
  • Knowledge of Interqual, Milliman, and Midas
