About the Role

We're seeking a highly skilled and compassionate Physical Therapist to join our team of clinical professionals. As a Home Health Physical Therapist, you'll play a vital role in developing and implementing personalized treatment plans to help patients achieve their maximum potential.

Key Responsibilities
  • Plan and administer prescribed physical therapy treatment and training to patients with various injuries, illnesses, and functional disabilities.
  • Conduct thorough assessments to evaluate patients' physical strengths and develop measurable training objectives.
  • Collaborate with physicians and clinical team members to develop, implement, and revise therapy treatment programs.
  • Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides, ensuring seamless care delivery.
  • Accurately document patients' care observations, interventions, and evaluations, ensuring timely submission for physician signature and implementation.
Requirements
  • Doctor of Physical Therapy (DPT) degree from an institution accredited by the American Physical Therapy Association (APTA).
  • Current and unrestricted license to practice Physical Therapy in the State of California.
  • Current CPR/BLS certification.
  • Compliant with state healthcare regulatory requirements.
  • Minimum 1 year of recent Physical Therapy experience in an institutional setting and/or home health agency.
  • Strong skills in observation, verbal and written communication, problem-solving, and basic computer skills.
