Key Responsibilities- Participate in the development and implementation of resident care plans, ensuring that they are tailored to meet the unique needs and preferences of each individual.
- Collaborate with the interdisciplinary team to review, plan, coordinate, and evaluate resident care, ensuring that all aspects of care are addressed and that residents receive comprehensive support.
- Document resident condition and nursing needs accurately and in a timely manner, reporting pertinent observations and reactions to the care team.
- Develop, direct, and monitor nursing assistant assignments, adjusting as needed to ensure that residents receive the highest level of care.
- Execute treatments as necessary, documenting reactions to medications and treatments, and initiating physician orders as required.
- Facilitate communication with families regarding changes in medications and/or changes in resident condition, ensuring that they are informed and engaged in the care process.
- Establish protocols to ensure that care plans are continually reviewed and updated to reflect the individuality of each resident.
- Coordinate admissions, discharges, and transfers, ensuring that the transition process is smooth and that residents receive the highest level of care.
- Graduate of an accredited nursing program.
- Certifications and licensure required by state regulations.
