About the Role
We are seeking an experienced Registered Nurse to join our team. The successful candidate will have a strong background in nursing, excellent communication skills, and a passion for providing high-quality patient care. In this role, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care coordination.
Responsibilities
* Conduct thorough assessments of patient needs, including physical, emotional, and social factors.
* Develop and implement individualized care plans that address patient goals and outcomes.
* Collaborate with physicians and other healthcare professionals to ensure comprehensive care coordination.
* Provide education and support to patients and their families on health-related topics and self-care strategies.
* Participate in interdisciplinary team meetings to discuss patient progress and develop care plans.
Requirements
* Graduate degree in Nursing from an accredited institution.
* Current licensure as a Registered Nurse in New York State.
* 2+ years of experience in a healthcare setting, preferably in a similar role.
* Excellent communication and interpersonal skills.
* Ability to work effectively in a fast-paced environment.
