Job Summary

Potomac Community Services is seeking a highly motivated and compassionate Law Enforcement Assisted Diversion Case Manager to join our team. As a key member of our community outreach team, you will work collaboratively with law enforcement, healthcare providers, and community organizations to provide harm reduction services, peer support, and case management to individuals at risk of law enforcement contact.

Key Responsibilities
  • Collaborate with law enforcement, healthcare providers, and community organizations to provide harm reduction services and case management
  • Provide peer support and connect individuals with community resources and services
  • Assess client needs and develop individualized care plans
  • Document client interactions and progress in electronic health records
  • Participate in regular team meetings and training sessions to stay up-to-date on best practices and program developments
Requirements
  • Bachelor's degree in human services, psychology, or related field
  • Minimum 2 years of experience in behavioral health, preferably working with adults
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
