Job Summary:
BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team as a Home Health Care Specialist. As a Physical Therapist, you will be responsible for providing high-quality patient care and developing individualized care plans to promote the health and well-being of our clients.
Responsibilities:
- Conduct home visits to assess and evaluate patient needs and develop personalized care plans.
- Perform diagnostic tests and measurements to identify patient needs and develop interventions to promote patient health and well-being.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assess and revise patient care plans to ensure optimal patient outcomes.
- Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote patient health and well-being.
- Accurately document patient observations, interventions, and evaluations in a state-of-the-art electronic medical record system.
Qualifications:
- A current New Jersey Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
