We are seeking a highly skilled Home Health Physical Therapist to join our team. With a strong commitment to patient care and a passion for delivering high-quality services, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.

Responsibilities- Evaluate patient functional status and physical therapy needs, assisting physicians in developing comprehensive plans of care.
- Conduct thorough assessments and evaluations to identify areas of improvement and develop targeted interventions.
- Develop and implement individualized treatment plans that cater to the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and maximize patient outcomes.
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification required.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
We offer a competitive salary range of $80,000 - $110,000 per year, depending on experience. Additionally, our comprehensive benefits package includes holiday pay, medical benefits, dental benefits, vision benefits, 401(k) retirement plan, continuing education opportunities, and employee assistance programs.
