Job Summary

We are seeking a highly skilled Occupational Therapist to join our interdisciplinary team at Center for Elders' Independence. As a key member of our PACE program, you will provide restorative and rehabilitative services to our elderly participants, promoting autonomy, quality of life, and independence in their communities.

Key Responsibilities
  • Collaborate with our interdisciplinary team to evaluate participants' level of functioning and potential for rehabilitation or restoration.
  • Develop and implement individualized plans of care, incorporating devices, techniques, and activities to promote muscle re-education, perceptual-motor and cognitive function, balance, endurance, and other factors affecting independence in daily living activities.
  • Conduct initial assessments and reassessments as required, and treat participants to relieve pain, develop or restore function, and maintain maximum performance in daily living activities.
  • Work with vendors to obtain rehabilitative equipment and supplies for participants.
  • Observe and record participants' responses to occupational therapy interventions and report regularly to the interdisciplinary team on any changes in condition.
  • Train family members, caregivers, and CEI staff in methods to maximize safety and functional abilities in performing daily living activities.
  • Attend interdisciplinary team conferences and participate in the development of the plan of care.
  • Evaluate participants' home environments and make recommendations as needed.
  • Participate in coordination for 24-hour care delivery to participants.
  • Delegate to and monitor the care provided by Occupational Therapy Assistants as appropriate and in accordance with California regulations.
  • Document thoroughly, accurately, and timely, and in accordance with Occupational Therapy practice standards and CEI policy.
Qualifications
  • Possession of a minimum of a Bachelor's degree and graduation from a program of Occupational Therapy approved by the Accreditation Council for Occupational Therapy Education.
  • Current licensure as an Occupational Therapist by the Occupational Therapy Board of California.
  • Preferred, one year of experience working as a Licensed Occupational Therapist with the frail elderly population.
  • CPR and First Aid certification on file.
  • A valid California driver's license, current vehicle insurance, and access to a reliable vehicle.
