Job SummaryWe are seeking a highly skilled Registered Nurse to join our Home Health - Hospice team. As a Registered Nurse, you will be responsible for managing and coordinating the care of patients in their homes, providing individualized care to patients, families, and caregivers.
Key Responsibilities- Evaluate and initiate appropriate care plans based on established standards.
- Provide patient and family education to support recovery and engagement in the care process.
- Regularly assess and document patient progress and responses to interventions.
- Coordinate care activities with other members of the healthcare team.
- Ensure accurate and efficient execution of physicians' orders.
- Timely communication of adverse or sudden changes in patient condition.
- Recognize and report adverse drug reactions.
- Effectively manage patient pain and utilize blood products as necessary.
- Safely and effectively utilize patient care equipment.
- Collaborate effectively across organizational boundaries.
- Adhere to the core values and standards of the organization.
- Graduate from an accredited nursing program.
- Current RN Washington State license.
- Current BLS.
- ACLS - within 3 months of hire.
- ENCP or PALS - within 6 months of hire.
- TNCC - within 1 year of hire.
- Oral and written fluency in the English language.
