About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Kadlec Regional Medical Center. As a Clinical Pharmacist, you will play a critical role in ensuring optimal use of medications to achieve specific outcomes that improve a patient's quality of life.

Key Responsibilities
  • Collaborate with the interdisciplinary team to provide chronic disease state management to patients
  • Initiate, modify, and discontinue medication therapy based on patient history, medical records, and barriers to access
  • Monitor medication therapy for side effects, effectiveness, and allergies
  • Provide drug information to patients and professional staff
  • Document patient care processes within the electronic health record
Requirements
  • Bachelor's Degree: Graduate from an accredited School of Pharmacy with a BS/PharmD degree or equivalent education/experience
  • Washington State Pharmacist License upon hire
  • One (1) year of PGY-1 residency or equivalent experience
Preferred Qualifications
  • Two (2) years of PGY-2 AmCare residency or equivalent experience
  • Certification in Ambulatory Care upon hire
