Care Coordinator

We are seeking a highly motivated and detail-oriented Care Coordinator to join our team at the YMCA of Greater Seattle. As a Care Coordinator, you will play a critical role in supporting our care coordination process, connecting individuals with our chronic disease prevention and management programs.
Key Responsibilities:- Maintain Confidentiality and Data Integrity: Maintain an electronic database of private health information and ensure that all data is entered and sent/received through the referral management system in accordance with all privacy and security protocols.
- Care Coordination Conversations: Conduct care coordination conversations with referred individuals and other prospective community members using motivational interviewing and active listening skills to promote healthy behaviors and support individuals in achieving their health goals.
- Marketing and Retention: Assist with marketing and retention activities to create robust classes with high enrollment, ensuring that our programs are accessible and appealing to diverse populations.
- Program Documentation: Ensure that accurate intake forms, data collection forms, and program documentation are completed, and that all program/enrollment targets are met and consistent with program standards.
- Quality Assurance and Administrative Support: Support quality assurance and other administrative duties related to chronic disease programming across the Association, ensuring that our programs are delivered with excellence and integrity.
- Education and Experience: One or more years of education and/or work experience in health promotion, public health, community outreach, or a similar field.
- Knowledge and Skills: Working knowledge of preventative health programs and philosophy, comfort interacting with individuals' health-related information, and ability to communicate with healthcare providers.
- Technical Skills: Intermediate to advanced computer skills and experience with Microsoft Office suite.
- Certifications and Training: YMCA Healthy Lifestyles Principles certification, Listen First Level I and II training, and current state-approved first aid and CPR for the Professional Rescuer certifications.
- Education and Experience: One or more years of college education in health promotion, public health, or a related field, or two or more years of experience working in health promotion, public health, or community outreach.
- Knowledge and Skills: Prefer knowledge of and previous experience with diverse populations, including language, culture, race, physical ability, and sexual orientation.
