Job Summary

Key Responsibilities
  • Conduct comprehensive medication reviews to identify potential medication-related problems and make evidence-based recommendations to optimize patient outcomes.
  • Collaborate with healthcare teams to develop and implement medication therapy plans that align with patient needs and goals.
  • Provide education and consultation to patients, families, and healthcare professionals on medication use, side effects, and interactions.
  • Participate in quality improvement initiatives to enhance medication use and patient safety.
  • Stay up-to-date with the latest clinical guidelines, research, and regulatory requirements to ensure compliance and best practices.
Requirements
  • Graduate of an ACPE-accredited School of Pharmacy with a Bachelor's of Science in Pharmacy.
  • Doctor of Pharmacy (Pharm D) degree preferred.
  • Completion of a post-graduate residency and/or board certification preferred.
  • Current licensure to practice in the state of NH and willingness to seek license reciprocity in multiple states if required.
Preferred Qualifications
  • Board Certification in Specialty Pharmacy (CSP) within 3 years of hire.
  • BLS Certification within 30 days of hire if required to administer immunizations.
