We are seeking a highly skilled Clinical Pharmacist-Inpatient to join our team at Dartmouth-Hitchcock Health. As a key member of our inpatient pharmacy team, you will play a critical role in ensuring the safe and effective use of medications for our patients.
Key Responsibilities- Conduct comprehensive medication reviews to identify potential medication-related problems and make evidence-based recommendations to optimize patient outcomes.
- Collaborate with healthcare teams to develop and implement medication therapy plans that align with patient needs and goals.
- Provide education and consultation to patients, families, and healthcare professionals on medication use, side effects, and interactions.
- Participate in quality improvement initiatives to enhance medication use and patient safety.
- Stay up-to-date with the latest clinical guidelines, research, and regulatory requirements to ensure compliance and best practices.
- Graduate of an ACPE-accredited School of Pharmacy with a Bachelor's of Science in Pharmacy.
- Doctor of Pharmacy (Pharm D) degree preferred.
- Completion of a post-graduate residency and/or board certification preferred.
- Current licensure to practice in the state of NH and willingness to seek license reciprocity in multiple states if required.
- Board Certification in Specialty Pharmacy (CSP) within 3 years of hire.
- BLS Certification within 30 days of hire if required to administer immunizations.
