LHC Group is seeking a skilled Registered Nurse to join their team as a Field Assessor in Home Health. The ideal candidate will have a strong background in nursing and experience in home health care.
Responsibilities:
The successful candidate will be responsible for completing Long Term Care insurance assessments, obtaining claimant demographics, physician information, and evaluating the claimant's functional level of independence. This will involve completing a brief cognitive screening and evaluating the claimant's ability to complete ADL and IADL tasks.
Requirements:
The ideal candidate will have a current, unencumbered RN licensure in the state of practice, a minimum of 2 years of home health experience or 2 years of nursing experience in an equivalent area of practice serving the older adult population, and be willing to obtain a professional liability insurance policy. Additionally, the candidate must have a current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
