Registered Nurse Per Diem Field Clinician Middlesex and Mercer Counties NJ

We are seeking a skilled Registered Nurse to join our Senior Community Care team as a Per Diem Field Clinician. In this role, you will provide care to our highest-risk health plan members in a facility setting, working collaboratively with an interdisciplinary team to coordinate clinical care and support patient needs.
Key Responsibilities- Ensure accurate and complete documentation of patient care and services provided
- Communicate and collaborate with the interdisciplinary care team to coordinate patient care and services
- Assist the provider team with various care coordination activities, including benefit determination and therapy coordination
- Participate in facility-based meetings and schedule and participate in family conferences, team meetings, and team case presentations
- Coordinate training, tracking, and compliance with quality measures
- Assist in creating and conducting in-services for selected audiences
- Associate degree or higher in Nursing (RN)
- Active and unrestricted license in the state where you reside, or ability to obtain by start date
- Certified in Basic Life Support
- Ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessment
- Access to reliable transportation
- Ability to travel in between facilities
- 1+ years of clinical experience in practice (acute care setting)
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
- Saline Lock insertion experience
- Understanding of Geriatrics and Chronic Illness
