Director of MDS Job Description

We are seeking a highly skilled and compassionate Director of MDS to join our team at Thornton Care Center. As a key member of our care team, you will be responsible for conducting and coordinating the development, completion, and transmission of the resident assessment in accordance with state requirements and facility policies.

Key Responsibilities:
  • Conduct and coordinate the development and completion of preliminary and comprehensive resident assessments (MDS) in accordance with current rules, regulations, and guidelines.
  • Ensure that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, mental, and psychosocial needs.
  • Coordinate the interviewing of each resident for the assessment and help develop and update a comprehensive resident assessment and care plan.
  • Evaluate each resident's condition and pertinent medical data to determine any need to amend the admission assessment.
Requirements:
  • 2-4 years' experience providing direct care to multiple patients/residents, preferably in a Skilled Nursing environment.
  • Nursing degree from an accredited college or graduation from an approved LPN program.
  • Current, unencumbered license to practice as an LPN in Colorado.
  • Current valid CPR certification.
