- Manage care coordination for a defined patient demographic within the hospital, ensuring effective resource utilization and adherence to external review standards.

- Enhance patient management quality and satisfaction, promoting seamless care continuity and cost efficiency through integrated care management.
Qualifications
Essential Requirements:
- Must possess a valid Registered Nurse license in Texas or be a Licensed Social Worker.
- Completion of educational requirements for Texas licensure as a Registered Nurse or Licensed Social Worker with a minimum of three years of case management experience.
- At least two years of nursing experience in an acute care or surgical environment is mandatory.
- Strong understanding of community resources, discharge planning, and behavioral health assessment skills.
Preferred Qualifications:
- Prior experience in case management is highly desirable.
