Job Summary

Community Health Network is seeking a skilled Registered Nurse to join our team. As a Nurse Care Specialist, you will be responsible for providing exceptional care to patients with wounds that are not healing properly. Your expertise in wound care and ability to work collaboratively with our team will make a significant impact on our patients' lives.

Key Responsibilities
  • Manage patient care in the healthcare continuum to achieve optimum outcomes in a safe and cost-effective manner
  • Perform intakes (initial assessments, charting) and discharges (instructions, dressings) for wound care patients
  • Function in a case management role to ensure seamless care coordination

Requirements
  • Demonstration of performance consistent with professional standards of practice, care, the Nurse Practice Act, and the mission and goals of Community Health Network
  • Demonstration of skills in collaboration, delegation, negotiation, and management of resources
  • Graduate of a National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC)
