Position Overview
Elderwood at Williamsville is seeking a dedicated and skilled Physical Therapist to provide exceptional care to our residents. This role is vital in enhancing the quality of life for individuals through comprehensive therapy services.

Role Responsibilities:
The Physical Therapist will:
- Implement skilled physical therapy interventions as per physician directives while maintaining consistent communication with the medical team.
- Document evaluations, progress notes, and discharge summaries for patients undergoing therapy.
- Collaborate with healthcare professionals to address the unique needs of each resident, promoting effective rehabilitation.
- Participate in team meetings and provide insights during the absence of the Director of Physical Therapy.
- Maintain accurate statistical records of patient treatments.
- Assist in training programs for staff as needed.
- Update and manage individual care plans in the Inter-disciplinary Kardex.
- Request necessary supplies for therapy sessions from the Director of Physical Therapy.
- Oversee the Physical Therapy Aides and ensure all documentation is completed in the absence of the Director.
Qualifications:
The ideal candidate will possess:
- A degree from an accredited physical therapy program.
- A valid state license to practice physical therapy.
- Preferred experience of two years in a physical therapy setting.
- Strong leadership skills to guide and direct others.
- Proficient communication skills in English, both written and verbal, to ensure effective interaction with residents and staff.
