As a Clinical Care Specialist, you'll play a vital role in our House Calls product, providing annual clinical assessments to patients in the comfort of their homes. This preventive care helps identify and reduce health risks, coordinating follow-up care to improve overall well-being.

Key Responsibilities:
  • Conduct in-home assessments, including medical history evaluation, medication reconciliation, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as needed.
  • Identify diagnoses for care management and active medical management.
  • Communicate findings to inform members' primary care physicians of potential care gaps.
  • Educate members on disease processes, medication, and compliance.
Requirements:
  • Active NP or PA license or ability to obtain by start date.
  • Active national NP or PA certification or ability to obtain national certification in the state of assignment.
  • Ability to complete physical job requirements, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Access to reliable transportation for up to 85% travel within licensed states.
