The Role
Pine Park Health is in search of part-time or full-time Primary Care Providers to deliver care in our East Bay market. If you are enthusiastic about managing a smaller patient panel of complex care cases, contributing to a value-based care model, and collaborating with a dedicated team to provide exceptional patient care, you are in the right place. This patient-centered care model aims to leverage the expertise of our care navigators to optimize the time and efforts of our providers.
Key Responsibilities
- Deliver on-site primary care services to elderly individuals residing in Senior Living Communities.
- Develop and implement comprehensive, person-centered care plans for all primary care patients.
- Collaborate with the Medical Director to establish and refine policies addressing daily practice issues, culture, or clinical guidelines, including emerging diseases.
- Coordinate with Primary Care Coordinators (PCCs) to conduct monthly Chronic Care Management (CCM) visits and oversee CCM documentation.
- Partner with Traveling Clinical Assistants (TCAs) to fulfill diagnostic requirements for each patient in the communities.
- Facilitate regular team meetings and provide leadership to the clinical staff.
- Identify collaborative opportunities with senior living management and care teams to enhance the execution of Pine Park care plans and improve patient outcomes.
Qualifications
- Board-Eligible or Board-Certified Physicians in Internal Medicine or Family Medicine, or Nurse Practitioners or Physician Assistants, preferably with experience in geriatrics.
- Adaptability to a fast-paced and evolving environment.
- Willingness to provide on-site clinical care across the East Bay region.
- Ability to travel to partner communities as necessary.
- Passion for working with complex care patients and supporting the development of a holistic care model.
- Strong inclination towards teamwork and collaboration with a multidisciplinary team; values contributions from all clinical team members.
- Excellent written and verbal communication skills.
- Proficiency in utilizing technology-enabled care delivery tools to enhance patient care and team efficiency.

The salary range for full-time positions in this role is competitive and includes base salary, performance bonuses, sign-on bonuses, equity, and benefits. The final compensation package will depend on various job-related factors unique to each candidate.
