Key Responsibilities- Lead by Example: Demonstrate exceptional nursing skills and provide guidance and support to your team members.
- Assess and Plan Care: Conduct thorough assessments of residents' needs and develop individualized care plans to promote their physical, mental, and emotional well-being.
- Coordinate Care: Collaborate with the interdisciplinary team to ensure seamless care delivery and effective communication.
- Medication Management: Administer medications as prescribed and maintain accurate records.
- Monitor and Report: Continuously monitor residents' conditions and report any changes or concerns to the physician and other relevant staff members.
- Supervise and Develop: Oversee and mentor nursing assistants to ensure they are providing high-quality care and meeting the needs of our residents.
- Incident Reporting: Initiate and complete incident reports as necessary, following facility policies and procedures.
- Discharge Planning: Coordinate the discharge process for residents, ensuring a smooth transition to their next level of care.
- Education: Graduation from an accredited RN program.
- Licensure: Active Registered Nurse licensure in the state of Texas.
- Experience: Preferably 1+ year of experience working in a skilled nursing facility.
