About Comprehensive Patient Care: At Riverside Health System, we value the importance of comprehensive patient care. As a Comprehensive Patient Care Coordinator, you will work closely with patients, families, and healthcare teams to ensure seamless and coordinated care.

Key Responsibilities:

  1. Patient Assessment: Assess patient clinical and psychosocial needs and apply standards of practice to provide high-quality care in Radiosurgery.
  2. Care Coordination: Partner with patients and families to develop, implement, and evaluate plans of care, treatments, and procedures that contribute to optimal patient outcomes.
  3. Procedure Assistance: Assist in the procedure as directed by the physician, including assistance with head ring placement and nursing care and monitoring of the patient during the procedure.
  4. Collaboration: Collaborate with the interdisciplinary team in care coordination, including diagnostic test results prior to the procedure, and facilitate safe patient transitions to the next care setting as part of the plan of care.
  5. Education: Provide patient education prior to and after the procedure, assuming responsibility for individualizing and coordinating patient education, and discharge-specific education that includes family/caregiver.
  6. Documentation: Accurately complete all required clinical and quality documentation consistently per RHS standards and regulatory requirements.
  7. Critical Thinking: Use critical thinking skills to delegate and prioritize tasks appropriately to facilitate quality patient care and ensure patient care needs are met.

Qualifications:

  • Associates Degree, Professional Nursing (Required)
  • Bachelors Degree, Professional Nursing (Preferred)
  • 2 years RN experience in minimally invasive procedures (Preferred)

Certifications and Licenses:

  • Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) Upon Hire (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire (Required)
